Post-Acute Services Director – Skilled Nursing (Healthcare Sales)

Job Summary

The Post-Acute Services Director drives census growth, referral development, and market expansion for assigned skilled nursing centers. This role builds strong hospital and community relationships, manages complex patient transitions, and executes strategic healthcare sales initiatives. The Director promotes the center’s clinical programs, analyzes referral data, and ensures smooth admissions to position the facility as a preferred post-acute partner.

About the Role

We are seeking a results-driven Post-Acute Services Director with experience in skilled nursing, healthcare sales, and hospital-based referral development . This position is responsible for generating high-quality referrals, improving conversion rates, and strengthening relationships with hospitals, physicians, payers, and community providers. The Director plays a key role in supporting patient outcomes and advancing the facility’s growth strategy.

Key Responsibilities

Referral & Transition Management

Lead the referral, clinical review, and admission process to ensure seamless patient transitions into the skilled nursing setting.

Navigate managed care authorization processes and hospital discharge workflows.

Use CRM tools for timely follow-up, accurate documentation, and effective clinical handoffs.

Relationship Development & Market Growth

Build strong relationships with hospital case managers, physicians, and discharge planners to drive consistent referrals.

Develop 5–10 new referral contacts per month across hospital departments a nd other downstream opportunities.

Promote the center’s clinical capabilities—short-term rehab, complex medical care, respiratory services, wound care, memory care, and more.

Healthcare Sales & Business Development

Execute targeted sales strategies that highlight clinical strengths and differentiate the center in the marketplace.

Achieve a 60%+ conversion rate for clinically and financially appropriate referrals and show upward trends within 90–120 days.

Conduct regular hospital outreach, presentations, and follow-ups to increase brand visibility and referral volume.

Use market analytics, referral trends, and competitive insights to guide growth initiatives.

Conducat, at minimum, 3-4 sales calls a week.

Strategic Planning & Performance Management

Develop a comprehensive market strategy for each assigned center using data and performance metrics.

Meet or exceed sales and admission goals, including 5% admissions growth per reporting period.

Partner with clinical and operational leaders to ensure alignment between sales efforts and clinical programming.

Preferred Experience

Prior experience as a Clinical Liaison, Admissions Director, Hospital Liaison, Case Manager, or similar role .

Experience marketing specialized programs: cardiac rehab, orthopedic rehab, wound care, respiratory care, dialysis coordination, etc.

Knowledge of territory management, market analysis, and competitive positioning.

Bachelor’s degree in Healthcare Administration, Nursing, Business, Marketing, or related field.

Qualifications

Required Qualifications

2–5 years of experience in skilled nursing, post-acute care, hospital discharge planning, or healthcare admissions/marketing.

Proven success in healthcare sales, business development, or referral relationship management.

Track record of meeting measurable sales outcomes (conversion rates, census growth, admission targets).

Strong communication, relationship-building, and presentation skills.

Proficiency in CRM systems and data-driven decision-making.

Ability to travel locally for hospital visits, meetings, and community outreach. Ability to travel statewide for company meetings and education.
