Primary Care Nurse Care Coordinator

We provide primary health care through a range of options including prevention, assessment, diagnosis and treatment. Our team includes health professionals, educators and support workers representing both traditional Indigenous and contemporary models of care, and uses wholistic health and community development concepts.

This position is community based and supports a client-centered inter-professional team structure to ensure coordinated, wholistic care is provided to a defined population of WNHAC clients. The positions also provide nursing services within the professional scope of practice established by the College of Nurses of Ontario. In extenuating situations, or in the event of an emergency, they may be redeployed to ensure provision of essential services.

This position is part of the Ontario Nurses Association Bargaining unit.

NOTE: Traditional practices involve cultural ceremonies and burning sacred medicines including tobacco, sweetgrass, sage and cedar within the work setting.

PRIMARY DUTIES / RESPONSIBILITIES

  • Carry out assessment, nursing diagnosis, and treatment within the nursing scope of practice, including but not limited to assessing episodic issues, well-baby exams, chronic disease monitoring, wound care, immunizations, medication administration, non-emergent triage, and client education.
  • Engage in expanded clinical practice through approved training and competency development, practicing under established medical directives or clinical protocols. This may include cervical cancer screening and smoking cessation interventions (e.g., STOP program).
  • Collaborate with professional team members to ensure comprehensive wholistic care for clients, making referrals and seeking and/or providing consulting support as appropriate.
  • Support to the primary care providers with various care coordination tasks to ensure timely review of incoming diagnostics and consults, monitoring and coordination of age-related screenings, completion of health assessment forms, and facilitating the communication of non-critical information to the client.
  • Support the client intake process through completion of medical history data entry into CPP, including immunization records.
  • Collaboration with Home and Community Care staff and Community Health Nurses to support the delivery of care measures required by respective programs.
  • Maintain current and accurate medical records, reports and other documentation to ensure comprehensive, consistent care and services, and to meet all accountability requirements.
  • Participate in quality improvement, evaluation, and risk management activities, including contributing to practice improvement initiatives, incident reviews, and the use of clinical information to support safe, effective client care.
  • Contribute to the advancement of evidence-informed nursing practice through participation in program development activities, knowledge sharing, and support of primary care initiatives.
  • Active participation in organizational teams and committees.
  • Other duties as required. In extenuating situations or in the event of an emergency, may be redeployed to ensure provision of essential services .

QUALIFICATIONS

· Current registration as a RN with the College of Nurses of Ontario, in good standing;

· Willingness to work in full scope of practice, performing delegated acts through medical directives; experience in immunization administration, venipuncture, wound care and advanced nursing assessment skills an asset;

· Demonstrated ability to work independently and be an effective team member;

· Excellent interpersonal and communication skills – demonstrated ability to communicate with health care professionals, community members, traditional helpers, and leadership;

· Understanding of wholistic health and well being, demonstrated cultural competencies and experience working in an Indigenous organization; ability to understand and speak Anishinaabemowin a definite asset;

· Willingness and ability to travel, with significant community-based activity;

· Valid drivers’ license and own vehicle required;

· Proof of immunization in compliance with policy requirements is mandatory;

· Current criminal records/vulnerable sector check required;

· Commitment to ongoing training and professional development relevant to job requirements;

· Positive attitude and capacity to act as a healthy lifestyle role model.

Salary: $38.48-$51.89 per hour with excellent benefits, including HOOPP pension plan. Salary is in accordance with the Ontario Nurses’ Association Contract (ONA).
