Job Title: Registered Nurse (RN) Case Manager – Home Health | See the Progress, Not Just the Shift Company: Home Health Care, Inc. What You’ll Do • Manage a home health caseload across homes and community settings. • Complete OASIS assessments and individualized care plans. • Provide skilled nursing care, education, and coordination. • Adjust care plans based on patient progress and needs. • Collaborate with physicians and interdisciplinary teams. • Maintain accurate, timely documentation. What We’re Looking For • Active Minnesota Registered Nurse (RN) license. • 3+ years of RN experience, home health preferred. • Strong clinical judgment and care planning skills. • Ability to work independently in a field-based role. • Reliable transportation.
