The Pharmacy Technician – Medication Historian plays a critical role in supporting medication safety and reducing hospital readmissions by ensuring accurate medication histories for patients presenting to the Emergency Department at Fairview Hospital, one of Cleveland Clinic’s largest regional hospitals. Working primarily within the ED, this caregiver conducts detailed patient interviews, reviews medication regimens, and verifies medication information through communication with patients, caregivers, retail pharmacies, and other healthcare providers. Responsibilities include completing and documenting comprehensive medication histories, updating medication lists within the electronic medical record, identifying discrepancies, and communicating findings to pharmacists, physicians, nurses, and other members of the care team. By providing accurate medication information at the point of admission, this role helps ensure safe medication ordering, supports continuity of care, and contributes to improved patient outcomes across the hospital.

A caregiver in this position will work Monday through Friday from 11:00am to 7:30pm on 2nd shift. No weekend or holiday expectations.

A caregiver who excels in this role will:  

  • Communicate directly with patients and caregivers while performing technical functions within the assigned work area to ensure timely and accurate procurement of prescriptions and documentation within the electronic medical record when required.  
  • Identify potential medication-related access problems and work with patients and the multi-disciplinary team to solve these problems in an efficient and patient-centered manner. 
  • Coordinate education, bedside medication delivery, and medication payment. 
  • Assist with the development and implementation of policies and procedures to ensure a seamless medication-related procurement process for patients. 
  • Work with the pharmacy leadership teams to identify areas of opportunity for improved patient experience.  
  • Assist manager in creating, implementing and monitoring quality control programs in assigned area. 
  • Conduct third party prior authorization requests when needed. 
  • Monitor program performance, including patient volumes served and patient experience indicators, serving as a valued resource to identify discharge prescription coverage issues and assist in their resolution. 
  • Complete medication histories or other transitional care activities as assigned per location. 

Minimum qualifications for the ideal future caregiver include:  

  • High School Diploma/GED  
  • Minimum six months related pharmacy experience (successful completion of a Pharmacy Technician Program may substitute for the required work experience)

Must meet all of the following license and certification combination requirements at the time of application:

  • Must hold an active registration as a Registered Pharmacy Technician through the Ohio Board of Pharmacy (OBOP).

  • It is required, within 18 months from the date of hire: Must obtain a national certification as a Certified Pharmacy Technician (CPhT) certification through the Pharmacy Technician Certification Board (PTCB) or the National Healthcare Association (NHA)  AND Registration as a Certified Pharmacy Technician with the Ohio Board of Pharmacy

Preferred qualifications for the ideal future caregiver include:

  • Completion of a formal pharmacy technician training program
  • Experience dealing with 3rd party insurance and prior authorization process
  • Certified Pharmacy Technician (CPhT) through the Pharmacy Technician Certification Board (PTCB) is preferred at time of hire

 

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.

  • Manual dexterity sufficient to perform computer and dispensing functions of the job.

  • Ability to lift up to 20 pounds.

  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
