SUMMAR Y
•
The Program Lead, under the supervision of the Regional Program Director or Assistant Director,
assists in oversight of the program and ensures the client-centered care and case coordination
and/or resource linkage to clients is done in a competent, responsible, and ethical manner.
ESSENTIAL DUTIES AND RESPONSIBILI TIES
•
Assessing clients on intake as needed, completing on-call duties, and ensuring Clarvida approved
behavioral health therapeutic services are being provided to clients and families to include but
not limited to: coping skills, behavior modification, parenting skills, and crisis intervention. Lead
will ensure that internal and external collaborations are occurring with behavioral
health/medical providers .

Working with Care Coordinators to ens ure that all assigned clients’ treatment team are
collaborating to ensure quality care to the client. Ensures that Care Plans and documentation
are consistent with Clarvida policy, state licensing regulations, and COA accreditation
requirements .

Ensuring that all clinical documentation has been completed within established time frames and
rectifying all deficits within the allotted time frames; monitoring monthly and quarterly reports.
Program Lead will monitor and work with Care Coordinators to close gaps in care (HEDIS) for
clients that have open gaps. Lead will monitor progress of closing gaps on a weekly basis.
Ensuring/conducting weekly supervision with Care Coordinators according to the specifications
contract and COA requirements.
Facilitating Treatment Team meetings and reviewing presentations on each assigned client as
required. Attending all quarterly director meetings as well as regional leadership meetings.
Supporting fellow team members to ensure the best outcomes for all Clarvida clients and
families.
Contributing to the Performance Improvement Cycle, including the collection and use of data to
continuously improve client and program services, using the Performance and Quality
Improvement Plan and other means to ensure integrity and excellence of services being
provided as well as documentation. Ensuring PQI activities are completed accurately and in a
timely manner, including Peer and PQI audits. Participating in MCO evaluations.

Manages and/or assists leadership team in overseeing client census and productivity
standards/requirements for Care Coord inators.
Responsible for assisting in program development, networking, and collaboration between
program areas.

SUPERVISORY RESPONSIBILIT IES

Routine supervision of up to eight (8) assigned Care Coordinators.
Assures the completion of supervisory responsibilities in accordance with the organization's
policies and applicable laws, potentially including but not limited to interviewing, hiring, and
training employees; planning, assigning, and directing work; appraising performance; addressing
complaints and resolving problems.

REQUIRED EDUCATION AND/OR EXPERI ENCE
•
Bachelor’s degree in a Human Service discipline from an accredited four -year college or
university and at least 3 years of relevant experience in the Mental Health field.
PREFERRED EDUCATION AND EXPERIENCE
•
Experience working with children/adolescents in a therapeutic, community-based treatment
environment .
CERTIFICATES, LICENSES, REGISTR ATIONS
•
Be a licensed driver with automobile insurance in good standing and access to reliable

QUALIFICATION S

Ability to read, analyze, and interpret the most complex documents. Ability to respond
effectively to the most sensitive inquiries or complaints. Ability to write speeches and articles
using original or innovative techniques or style. Ability to make effective and persuasive
speeches and presentations on controversial or complex topics to top management, public
groups, and/or boards of directors.

Ability to apply advanced mathematical concepts such as exponents, logarithms, quadratic
equations, and permutati ons. Ability to apply mathematical operations to such tasks as

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to
interpret an extensive variety of technical instructions in mathematical or diagram form and
