Job description

Position Summary

The Case Manager provides non-clinical case management support for Soldiers requiring medical readiness assistance. This position ensures accurate case tracking, documentation, and communication to support deployability and readiness objectives.

Work Schedule

  • Full-Time
  • 40 Hours per Week
  • Monday through Friday
  • 7:30 AM – 4:30 PM Eastern Time
  • On-site support required
  • Travel throughout Ohio may be required for SRP and Medical Readiness Events
  • Assigned support for approximately 9–15 readiness events annually depending upon brigade assignment

Responsibilities

  • Manage assigned Soldier medical readiness cases.
  • Coordinate follow-up care and appointments.
  • Maintain records within eCase, MEDPROS, HRR, and MEDCHART.
  • Process Release of Information (ROI) requests.
  • Conduct monthly case reviews.
  • Communicate with Soldiers, Units, MRNCOs, and medical personnel.
  • Support SRP and Medical Readiness Events.
  • Track and report medical readiness metrics.

Minimum Qualifications

  • Licensed Practical Nurse (LPN), Licensed Social Worker, or Bachelor's Degree in a health-related field.
  • Minimum one (1) year of case management experience.
  • Strong communication and organizational skills.
  • Proficiency with Microsoft Office applications.
