We are currently seeking licensed Physical Therapists to provide home health services across Oʻahu.

Key Responsibilities

• Provide skilled physical therapy visits in patients' homes
• Conduct PT evaluations and follow-up assessments
• Develop individualized treatment plans focused on mobility, strength, balance, and fall prevention
• Educate patients and family caregivers on safe mobility techniques
• Maintain accurate, Medicare-compliant documentation
• Collaborate with the care team to support patient progress

Qualifications

• Current Hawaii Physical Therapist (PT) license
• Minimum 1 year clinical experience (home health experience preferred)
• Strong documentation and communication skills
• Ability to work independently in a home setting
• Valid driver's license and reliable transportation
