Home Care Physical Therapist

University Home Care Inc is seeking a compassionate and skilled Home Care Physical Therapist to join our team in Livonia, MI. In this role, you will provide high-quality physical therapy services to patients in their homes, helping them improve mobility, manage pain, and enhance their overall quality of life.

Responsibilities

• Conduct in-home physical therapy evaluations and assessments
• Develop and implement individualized treatment plans based on patient needs
• Provide therapeutic interventions to improve patient mobility and function
• Monitor patient progress and adjust treatment plans accordingly
• Educate patients and their families on exercises and care techniques
• Maintain accurate and timely documentation of patient care
• Collaborate with healthcare providers to ensure continuity of care

Requirements

• Valid Physical Therapist license in the state of Michigan
• Experience in home health or outpatient physical therapy preferred
• Strong interpersonal and communication skills
• Ability to work independently and manage a flexible schedule
• Reliable transportation for travel to patient homes in the Southeast MI Region
• Commitment to providing compassionate patient-centered care
