Job Summary Provide physical therapy services in the home under the direction of a licensed PT Follow the physician plan of care to support patient progress Communicate with the care team to ensure quality and safety Use clinical judgment to deliver effective treatments Maintain accurate and timely documentation Follow all safety, compliance, and infection control standards
