Department: Acute Rehab
Schedule: PRN, Nights
What You Will Do
•
Deliver compassionate, high-quality nursing care by following established clinical standards and protocols to ensure optimal patient safety and well-being.
•
Champion individualized patient care plans by continuously monitoring progress and adapting interventions to meet the evolving needs of each patient.
•
Drive interdisciplinary collaboration as the primary care coordinator, aligning doctors, specialists, and support staff to deliver seamless, well-rounded patient care.
•
Advocate for smooth patient transitions by assessing and coordinating comprehensive discharge plans that prepare patients and their families for successful recovery at home.
•
Execute and manage clinical orders with precision, maintaining accurate, real-time records of the patient's condition to keep the entire care team informed.
What You Will Need
Licensure / Certification / Registration
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Education
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
