What You Will Do
- Oversee and execute personalized care plans to ensure every patient reaches their health goals safely.
- Maintain patient safety through vigilant monitoring and real-time communication of changing conditions to the medical team.
- Act as the central coordinator between all medical disciplines to ensure a seamless and unified treatment experience.
- Lead the discharge planning process to ensure patients and families are fully prepared for success after they leave our care.
What You Will Need
Licensure / Certification / Registration
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date required.
Education
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Additional Preferences
- 36 hours a week | Nights, 7 PM - 7:30 AM, every other weekend and rotating holidays
- Prior Acute Care Preferred
