Physician Assistant / Nurse Practitioner – Pulmonary Medicine
Location: Melville, NY with occasional coverage in Commack, NY Schedule: Full-Time, Monday–Friday, approximately 8:00 AM – 4:00 PM or 9:00 AM – 5:00 PM Department: Pulmonary Medicine Employment Type: Full-Time, Outpatient Only
About the Collaborating Physician
The APP will work closely with Dr. Rawinis, an experienced pulmonologist and sleep medicine specialist with Catholic Health who provides comprehensive care for adults with pulmonary and sleep disorders, including asthma, COPD, pulmonary fibrosis, sleep apnea, insomnia, and other respiratory conditions.
Dr. Rawinis is committed to patient-centered care, education, and shared decision-making, fostering a collaborative practice environment where APPs are valued members of the care team and supported in their professional growth and clinical development.
About the Practice
Join a well-established outpatient pulmonary practice providing comprehensive respiratory care to patients across Long Island. The Advanced Practice Provider will work collaboratively with a team of experienced pulmonologists while managing patients with a high degree of autonomy in a supportive and collegial environment.
Position Summary
The Advanced Practice Provider (Physician Assistant or Nurse Practitioner) will provide high-quality outpatient pulmonary care, supporting physicians in the evaluation, diagnosis, treatment, and longitudinal management of patients with acute and chronic pulmonary conditions.
This role offers the opportunity to practice independently with physician collaboration while contributing to a growing pulmonary practice focused on patient-centered care.
Responsibilities
- Perform comprehensive patient evaluations and follow-up visits in the outpatient setting.
- Assess, diagnose, and manage patients with pulmonary conditions including:
- Asthma
- COPD
- Pulmonary nodules
- Interstitial lung disease
- Sleep-related breathing disorders
- Chronic cough and other respiratory conditions
- Order and interpret diagnostic testing, laboratory studies, and imaging.
- Manage medications and treatment plans.
- Complete prior authorizations and medication management activities, including biologic therapies.
- Provide patient education regarding disease management, medications, and treatment plans.
- Collaborate closely with physicians and office staff to ensure continuity of care.
- Support providers with clinical documentation and care coordination activities.
Qualifications
Required
- Graduate of an accredited Physician Assistant or Nurse Practitioner program.
- Current New York State PA or NP license.
- National board certification.
- DEA registration or eligibility prior to start date.
- Current ACLS and BLS certifications.
Preferred
- Prior experience in pulmonary medicine strongly preferred.
- Experience managing biologic therapies and prior authorizations preferred.
- Previous outpatient APP experience required.

Additional Information
- DEA reimbursement available.
- Limited travel between Melville and Commack locations (approximately 25% travel requirement).
- Collaborative practice consisting of three pulmonologists and multiple APPs and support staff.
Posted Salary Range
USD $69.55 - USD $93.00 /Hr.
