Department: Case Management
Schedule: Full-Time | Days
What You Will Do
- Psychosocial Assessment: Evaluating complex emotional, psychological, and physical issues to determine the support needed for both patients and their families.
- Interdisciplinary Care Planning: Collaborating with physicians, nurses, and therapists to develop and reevaluate individualized care plans from admission through discharge.
- Patient Advocacy: Ensuring the medical system respects patient rights and preferences, especially in difficult cases involving ethics, end-of-life decisions, or abuse/neglect.
- Resource Coordination: Connecting patients with community resources for financial assistance, housing, food, and long-term care placement (e.g., skilled nursing or rehab).
What You Will Need
Licensure / Certification / Registration
- Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
- Certified Social Worker preferred.
Education
- Bachelor's degree in Social Work required.
Work Experience
- 3 years of experience preferred.
