Responsibilities

Job Summary

Provide a range of community and field-based prenatal outreach and care coordination to designated community sites in the Vista, Oceanside and San Marcos areas including outreach, health insurance education, and linkage to prenatal care. Assist in prenatal patient recruitment, program activities, and administrative support.

Essential Job Functions

  • Conduct community outreach, including giving presentations, and staffing at health fairs and events
  • Create partnerships with local high schools, rehab facilities and other community programs for recruitment of prenatal patients
  • Create outreach plans to increase prenatal program & CPSP enrollment
  • Contact community programs to present prenatal services available
  • Provide enrollment education
  • Instruct clients on the documents necessary for the eligibility process, and provide an overview of available healthcare options and services offered in the prenatal program
  • Refer clients to support services and programs - CBE, Lactation, CPSP
  • Compile statistics and reports on the number of outreach events, clients educated, clients referred to prenatal program and healthcare utilization of prenatal care with VCC
  • Serve as a liaison between VCC and community partners.
  • Participate in weekly and monthly staff meetings or as needed
  • Attend community meetings, events, health fairs and other outreach opportunities.
  • Organize and present prenatal education for community presentations
  • Complete all monthly activity reports at the end of each month, and on a quarterly basis
  • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
  • Support the vision, mission and goals, and demonstration commitment the values of the organization
  • Responsible to laptop, tablet and other IT items needed for offsite outreach

Qualifications

Minimum Qualifications

  • Bachelor's degree in related field
  • Minimum two years’ outreach and community education experience or equivalent combination of education and experience
  • Bilingual English/Spanish
  • Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
  • Availability and willingness to work evening and weekend hours, as required. Flexibility is a must.

  • Minimum four years‘ outreach and community education experience or equivalent combination of education and experience

Required Skills/Knowledge/Abilities

  • Comfortable with initiating conversation with various populations regrading prenatal program & services
  • Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Ability to prioritize and meet deadlines
  • Ability and willingness to meet the organization’s attendance and dress code policies
  • Ability to handle confidential materials and information in a professional manner
  • Excellent customer service skills and commitment to providing the highest level of customer satisfaction
  • Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel; Zoom

Salary Range

  • $23.00 - $25.00 per hour
