Location: Elk Grove Village, IL
Facility: Ascension Home Health
Department: Home Health Therapeutic Services
Schedule: Day shift | PRN | Flexible hours
Salary Range: $41.00 per hour | Base rate + visit differentials
What You Will Do
- Evaluate, treat, and provide speech‑language therapy to home health patients using appropriate therapeutic techniques.
- Educate patients and families on treatment plans, maintain clinical documentation, and follow performance improvement guidelines.
- Support discharge planning and connect patients with appropriate community or support programs.
What You Will Need
Licensure / Certification / Registration
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
- Speech-Language Pathologist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
Education
- Bachelor's degree required.
- Master's degree preferred.
Additional Preferences
Bachelors in Speech Pathology required.
Experience in geriatric and home health strongly preferred.
