Key Responsibilities
Clinical Nursing Care
- Perform comprehensive patient assessments, including vital signs monitoring, blood glucose checks, and diabetes-related evaluations.
- Assist in diabetes management, including insulin administration, medication management, and patient education.
- Support physicians and specialists during consultations, medical procedures, and treatments.
- Ensure accurate documentation of patient records, treatment plans, and follow-ups in the electronic medical record (EMR) system.
- Reporting & Accountability: Provide regular updates to the Lead Nurse regarding clinic flow, patient escalations, and any deviations from clinical protocols.
Data Collection & Research
- Collect and record patient health data, including blood glucose levels, weight, blood pressure, and other relevant health metrics.
- Analyze patient data trends to assist in early identification of complications and treatment adjustments.
Infection Control & Compliance
- Adhere to infection prevention protocols, safety standards, and regulatory guidelines as per DHA requirements.
- Ensure compliance with clinic policies, medication safety standards, and quality control procedures.
- Maintain proper handling and disposal of medical supplies, sharps, and biohazard materials.
- Incident Reporting: Immediately report any clinical incidents, near-misses, or patient grievances to the Lead Nurse in accordance with JCIA and DHA safety standards.
Accreditation & Quality Compliance
- Conduct monthly International Patient Safety Goals (IPSG) audits to ensure compliance with safety and quality standards.
- Actively participate in Joint Commission International Accreditation (JCIA) compliance efforts, ensuring adherence to global healthcare quality standards.
- Implement and monitor best practices that align with patient safety, risk management, and quality improvement initiatives.
- Competency Compliance: Proactively participate in annual nursing competency assessments and skills validations conducted by the Lead Nurse.
- Audit Support: Assist the Lead Nurse in gathering data for departmental audits, including medication fridge logs, equipment calibration checks, and inventory counts.
Awareness of Clinic Models & Programs
- Stay informed about GluCare’s unique care models and patient programs to enhance patient-centered services.
- Educate patients on clinic-specific treatment programs, lifestyle interventions, and digital health solutions.
- Ensure seamless integration of new clinical workflows in alignment with the clinic’s care philosophy.
Interdisciplinary Collaboration
- Work closely with doctors, diabetes educators, dietitians, and lifestyle coaches to deliver a comprehensive care experience.
- Actively participate in team meetings, case discussions, and quality improvement initiatives.
- Provide compassionate support to patients and families, ensuring a positive and comfortable clinic experience.
- DHA License as a Registered Nurse.
- Experience in diabetes management, chronic disease care, or digital health solutions is preferred.
- Strong knowledge of accreditation standards (JCIA), infection control, and patient safety protocols.
- Proficiency in using EMR systems and digital healthcare technologies.
- Ability to complete and send visit summaries accurately and efficiently to patients.
- Excellent communication and teamwork skills.
- Bachelor’s Degree or Diploma in Nursing from a recognized institution.
- Valid DHA license
- Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) preferred.
- 2+ years of clinical nursing experience (preferably in endocrinology, diabetes care, or chronic disease management).
- Experience in data collection, digital health, wearable devices, or patient education is an advantage.
