About the Role: We are seeking a dedicated and passionate Physical Therapist/PTA to join our team at One Health Home Health. As a key member of our team, you will have the opportunity to make a meaningful impact on the lives of our patients in Contra Costa, Alameda, and Solano Counties.
Responsibilities
- Develop and implement individualized treatment plans for patients
- Conduct home visits and provide hands-on therapy
- Educate patients and their families on exercises and techniques
- Collaborate with other healthcare professionals to ensure comprehensive care
- Document patient progress and communicate with the healthcare team
Requirements
- Valid Physical Therapist license in California
- Minimum of 1 year of experience in home health or a similar setting
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
- Passion for providing high-quality, patient-centered care
