340B Patient Care Coordinator

Empath Partners in Care , a part of Empath Health is seeking a 340B Patient Care Coordinator to join our team in St. Petersburg, FL . In this role, the 340B Patient Care Coordinator provides comprehensive care to clients and patients by assessing, planning, and advocating for their health care needs. The patient care coordinator will act as the liaison between EPIC's 340B program and the medical staff at their outposted healthcare facility. The 340B Patient Care Coordinator will monitor and coordinate patients' treatment plans, guide them to understand the status of their health, and check in on their progress. The Patient Care Coordinator also collaborates with other members of the care team to look after patients within the healthcare facility.

What You'll Do

• Educate patients on care coordination services, EPIC workflows, and the 340B program
• Coordinate comprehensive care, linking patients to EPIC and community resources
• Assist HIV patients with insurance navigation and healthcare cost support
• Provide ongoing education and support to newly diagnosed or returning patients
• Serve as liaison across departments to ensure 340B compliance and integrity
• Monitor and maintain knowledge of HRSA/OPA regulations and policy updates
• Support PrEP patients with appointment reminders, co-pay assistance, and pharmacy coordination
• Build relationships with local HIV/STD and social service agencies
• Provide emotional support and guidance to patients for treatment adherence
• Perform data entry, reporting, and analytical tasks

What You'll Need

• Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
• 2 - 3 years of experience in a health care setting preferred
• Minimum of one years' experience with 340B software, preferred
• Strong knowledge of Microsoft Suite products, specifically Excel
