Job Summary
Job Description SummaryThe Community Health Worker is responsible for providing advanced health education, outreach, and care coordination services to individuals and families in the community. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. They work closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes.
Primary Responsibilities:
- Conduct in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes.
- Provide advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions.
- Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs.
- Assist patients in navigating the healthcare system and accessing appropriate services, including health insurance and social services.
- Provide ongoing follow-up and support to patients to ensure continuity of care and successful achievement of health goals.
- Develop and implement health promotion programs and activities to address community health needs.
- Collect data and maintain accurate records of patient interactions and outcomes.
- Attend meetings and trainings related to community health promotion and education.
Qualifications
Qualifications
- Bachelor's Degree in Public Health, Social Work or related field of study required. Experience in lieu of degree can be accepted.
- 2+ years of experience in community health outreach, health education, or related field required.
- Valid Driver's License required.
- Bilingual (English/Spanish) preferred.
Additional Knowledge, Skills and Abilities:
- Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
- Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred.
- Ability to work independently and as part of a team.
- Basic computer skills, including Microsoft Office and database management.
- Bilingual skills preferred.

Schedule and Work Model
- Full-time, Monday through Friday, approximately 8am-4:30pm
- Hybrid work model: In-clinic work, remote work and field travel to meet with patients in the community as needed.
- On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams Video is required and must be accessed using MGB-provided equipment.
