Position Summary

The Hermann Center is seeking a Family Support and Care Coordinator (Case Worker/BSW) to provide intake support, care coordination, resource navigation, family support, and case management services for young children, pregnant and postpartum individuals, caregivers, and families receiving services through the Hermann Center and Synchrony programs.

This position serves as an important bridge between families, clinical providers, psychiatry, community resources, and support services. The Family Support and Care Coordinator will assist families in navigating behavioral health, developmental, educational, medical, child welfare, and social service systems while helping reduce barriers to treatment participation and supporting engagement in services.

The position will work closely with the Clinical Program Manager, therapists, psychiatrists, peer support specialists, community program staff, and research personnel to ensure that families receive coordinated, family-centered care. The role will be particularly important in supporting families involved with the County Services Fund (CSF), foster care, adoption, guardianship, court systems, and other community-based services.

As the Hermann Center continues to grow, this position may also provide caregiver support, parent education, and other family-focused services under appropriate supervision and training. The role offers opportunities for professional growth within an interdisciplinary academic medical center committed to early childhood mental health, family-centered care, innovation, training, and community impact.

The ideal candidate will be passionate about supporting young children and families, skilled in building relationships with caregivers, and knowledgeable about community resources and systems of care.

Job Description

Primary Duties & Responsibilities:

Intake, Screening, and Triage

  • Conduct intake interviews and gather psychosocial information from caregivers.
  • Assist with triage and determination of appropriate Hermann services.
  • Coordinate referrals to therapy, psychiatry, parent support, and community services.
  • Support intake processes for pregnant and postpartum individuals.
  • Assist with identifying urgent needs and connecting families with appropriate supports.


Family Support and Care Coordination

  • Provide family-centered support to caregivers.
  • Assist families in navigating healthcare, educational, developmental, and social service systems.
  • Provide support related to foster care, adoption, guardianship, and court-involved situations.
  • Facilitate communication among families and providers.
  • Assist families in addressing barriers to treatment participation.
  • Work closely with Peer Support Specialist.

Resource Navigation and Case Management

  • Connect families with community resources.
  • Assist with public benefits, housing, transportation, childcare, food security, and other social determinants of health.
  • Support County Services Fund (CSF) service delivery and documentation.
  • Maintain knowledge of community resources available to young children and families.

Program Support and Documentation

  • Track referrals and outcomes.
  • Maintain required documentation.
  • Assist with program evaluation and research activities.
  • Participate in multidisciplinary team meetings.
  • Support quality improvement initiatives.

Required Qualifications:

  • Basic Life Support certification must be obtained within one month of hire date
  • Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements). 
  • Equivalent of a bachelor’s degree in a health-related field, such as social work, counseling, sociology, or psychology. Applicants with bachelor’s degrees in other fields may be considered with sufficient case management experience.

The Hermann Center for Child and Family Development at Washington University School of Medicine provides innovative, relationship-based behavioral health services for young children, pregnant and postpartum individuals, caregivers, and families. The Center integrates clinical care, research, training, community partnerships, and dissemination efforts to improve the mental health and well-being of children during the earliest years of life.

The Hermann Center serves children from birth through age six and their caregivers through a range of evidence-based interventions, including infant and early childhood mental health services, parent-child therapies, parenting interventions, psychiatric services, and family support programs. The Center also provides specialized services for pregnant and postpartum individuals, foster and adoptive families, and children experiencing developmental, behavioral, emotional, and trauma-related concerns.

Salary Range

$20.57 - $30.84 / Hourly

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
