Clarity Healthcare is seeking a compassionate, motivated Community Health Worker (CHW) to help individuals overcome barriers to care and improve their overall health and well-being. In this role, you will serve as a trusted liaison between patients, healthcare providers, and community resources, helping individuals navigate complex systems, access essential services, and achieve their health goals.

As a Community Health Worker, you'll play a critical role in addressing social determinants of health, promoting health education, supporting care coordination, and empowering individuals to take an active role in their healthcare journey.

What You'll Do

Care Coordination & Patient Support

  • Serve as a bridge between patients, healthcare providers, and community-based organizations.
  • Assist individuals in navigating healthcare, behavioral health, and social service systems.
  • Identify barriers to care and collaborate with patients and care teams to develop solutions.
  • Support care management initiatives, including chronic disease management, preventative care, and patient education.
  • Conduct follow-up outreach through phone calls, community visits, and home visits as appropriate.

Community Outreach & Engagement

  • Build relationships within the community to identify and engage individuals who may benefit from healthcare services and support.
  • Educate community members on available healthcare resources, preventative care, and wellness initiatives.
  • Participate in outreach events and community engagement activities.
  • Advocate for patients and promote culturally responsive, person-centered care.

Resource Navigation & Advocacy

  • Connect individuals with appropriate healthcare services, support programs, and community resources.
  • Assist patients in understanding insurance coverage options and completing eligibility and enrollment applications, including Qualified Health Plans (QHPs).
  • Provide education and support that is culturally and linguistically appropriate for the populations served.
  • Encourage self-advocacy and empower individuals to make informed healthcare decisions.

Collaboration & Documentation

  • Work closely with providers, care teams, and community partners to coordinate services and improve health outcomes.
  • Maintain accurate documentation and data entry in accordance with organizational requirements.
  • Assist with departmental projects, quality initiatives, and other duties as assigned.

What You'll Bring

Knowledge, Skills & Abilities

  • Understanding of community health principles, social determinants of health, harm reduction practices, and motivational interviewing techniques.
  • Knowledge of local community resources, programs, and support services.
  • Strong customer service, relationship-building, and problem-solving skills.
  • Excellent interpersonal and active listening abilities.
  • Ability to communicate effectively with diverse populations in a clear, compassionate, and professional manner.
  • Strong organizational skills with attention to detail.
  • Proficiency with Microsoft Office applications, including Excel, Outlook, Teams, and Word.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.

Education & Experience

Required

  • High School Diploma or equivalent.
  • Valid driver's license, reliable transportation, and current auto insurance.

Preferred

  • Bachelor's degree in Health Sciences, Social Work, Human Services, Public Health, or a related field.
  • Two (2) years of experience in community health, patient navigation, healthcare outreach, human services, case management, or a related field.

Employment Requirements

  • Tuberculosis (TB) screening required for all direct care staff.
  • Complete an approved Community Health Worker training program through a recognized curriculum provider.
  • Complete required training related to:
    • Missouri Foster Care Program
    • Trauma-Informed Care
    • Behavioral Health Conditions in Children
    • Suicide Prevention
    • Substance Use Disorders
  • Obtain First Aid Certification within three (3) months of hire and maintain certification thereafter.
  • Obtain CPR Certification within three (3) months of hire and maintain certification thereafter.
