Registered Nurse (RN) Case Manager

We are seeking an experienced Registered Nurse (RN) Case Manager to support our centralized care management team. This role focuses on population health, chronic disease management, and care coordination within a Patient-Centered Medical Home (PCMH) model.

What We’re Looking For

• •A Bachelor’s of Science degree in Nursing (BSN) from an accredited institution. •A minimum of three (3) years of clinical nursing experience. •Knowledge of medical terminology, legal and technical aspects of a medical records system, HIPAA guidelines and ICD-10 and CPT codes. •Computer proficiency, particularly with Electronic Health/Medical Record systems and the Microsoft 365. •Bilingual in English and Spanish. Desired Qualifications:
• Case management certification is desirable.
• A minimum of two (2) years of experience in case management. •Experience in an outpatient setting or Federally Qualified Health Center (FQHC).
• Experience with the Epic electronic health record system.

The pay range for this role is $92,977 to $139,466 on an annual basis.
