We are seeking an experienced Registered Nurse (RN) Case Manager to support our centralized care management team. This role focuses on population health, chronic disease management, and care coordination within a Patient-Centered Medical Home (PCMH) model.
What We’re Looking For
- •A Bachelor’s of Science degree in Nursing (BSN) from an accredited institution. •A minimum of three (3) years of clinical nursing experience. •Knowledge of medical terminology, legal and technical aspects of a medical records system, HIPAA guidelines and ICD-10 and CPT codes. •Computer proficiency, particularly with Electronic Health/Medical Record systems and the Microsoft 365. •Bilingual in English and Spanish. Desired Qualifications:
- Case management certification is desirable.
- A minimum of two (2) years of experience in case management. •Experience in an outpatient setting or Federally Qualified Health Center (FQHC).
- Experience with the Epic electronic health record system.

The pay range for this role is $92,977 to $139,466 on an annual basis.
