As a Home Health/Hospice Coder, here's a big-picture view of what you'll do:
•
Review clinical documentation and OASIS/HOPE assessments to accurately assign ICD-10-CM diagnosis codes in accordance with CMS regulations and Official Coding Guidelines.
•
Ensure coding accuracy, completeness, and compliance for all assigned home health patient records.
•
Collaborate with clinicians, Quality Assurance, Clinical Managers, and Administrators to resolve documentation and coding questions.
•
Review documentation for coding opportunities and communicate recommendations to appropriate team members.
•
Generate coding reports and provide timely updates to leadership regarding coding status and productivity.
•
Support accurate reimbursement by ensuring documentation supports assigned diagnoses and regulatory requirements.
•
Participate in coding audits, quality improvement initiatives, and ongoing education to remain current with Medicare regulations, OASIS/HOPE guidance, and ICD-10 coding updates.
•
Maintain strict confidentiality of Protected Health Information (PHI) and comply with HIPAA privacy and security regulations.
•
Assist with special projects and perform other duties as assigned.
A few must-haves for our Home Health/Hospice Coders
•
High school diploma or GED required; Associate's degree in Health Information Management, Medical Coding, or a related field preferred.
•
Minimum of three (3) years of medical coding experience in a Medicare-certified home health/hospice setting.
•
Demonstrated experience with ICD-10-CM coding, OASIS/HOPE review and coding, and Medicare home health reimbursement.
•
Current coding certification preferred, including HCS-D, HCS-O, CPC, CCS, COS-C , or equivalent.
•
Strong knowledge of medical terminology, anatomy, and home health documentation requirements.
•
Thorough understanding of CMS Conditions of Participation, Medicare regulations, and home health coding guidelines.
•
Exceptional attention to detail, analytical skills, and commitment to coding accuracy.
•
Ability to manage multiple priorities, meet deadlines, and work independently in a fast-paced environment.
•
Excellent written, verbal, and interpersonal communication skills.
