Position Overview Elderwood at Lancaster is seeking a compassionate and dedicated Physical Therapist to join our skilled nursing facility team. Our Physical Therapy professionals play a vital role in ensuring the health, safety, and well-being of our residents by providing high-quality, individualized therapy services that support recovery, mobility, and independence.

Responsibilities

Provide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting

Develop and implement individualized treatment plans based on resident needs and physician recommendations

Administer skilled physical therapy interventions to improve mobility, strength, balance, and overall function

Monitor resident progress and adjust treatment plans as appropriate

Collaborate with interdisciplinary team members to ensure coordinated and effective care

Maintain accurate and timely clinical documentation in accordance with regulatory standards

Ensure the highest degree of quality care and resident safety is maintained at all times

Qualifications

Current State Physical Therapy License required

Strong clinical assessment and treatment planning skills

Ability to work collaboratively in a multidisciplinary team environment

Commitment to providing compassionate, resident-centered care

Responsibilities

Physical Therapist

Initiates skilled physical therapy treatments in accordance with physician’s orders; maintains required contact with physician.

Maintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program.

Confers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.

Provides input for meetings and attends during absences of the Director Physical Therapy.

Maintains statistical records of individuals’ treatments.

Assists with in-service training programs upon request.

Records and updates individual’s plan of care in the Inter-disciplinary Kardex.

Submits requisition for supplies needed in the treatment to Director Physical Therapy.

Assumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation.

Qualifications

Physical Therapist

  • Graduation from an approved school of physical therapy required.
  • State license to practice physical therapy required.
  • Two years’ experience in physical therapy preferred.
  • Ability to direct others necessary.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
