Job description

Job Requirements

Under general supervision, visits individuals and families in their homes, in shelters or other similar places to provide basic community outreach services and to assist with programmatic interventions necessary in coping with the transition from hospital to community, or reducing the risk of being admitted to the hospital. Help to train, mentor, and provide direction to CHW I when assigned and applicable. Assist with special project and training new potential Community Health Worker hires.

Responsibilities and Tasks

  • Conduct home visits to validate program interventions and communicates findings routinely to care coordinator team members.
  • Obtain routine information and reports to licensed team members as directed.
  • Complete documentation in database and other systems regarding patient or role specific activities.
  • Assist with the identification of community resources on an ongoing basis.
  • Communicate general information to individuals and families on program objectives and services, eligibility requirements and benefits, confidentiality of information, etc. Distributes informational materials and literature.
  • Remind patients and attend scheduled medical or community resource appointments as needed.
  • Participate in identified Transitional Care program meetings, and provides patient information regarding status of plan.
  • Communicate changes to schedules, application processes, co-payments required, and pertinent nsurance information.
  • Prepare written reports and maintain records of outreach contacts and activities.
  • Update information in database from various data forms collected.

Work Experience

Education and Experience

  • High School Diploma is required. Associates Degree is preferred.
  • Community Healthcare Worker Certification is required.
  • Minimum 2 years field experience.
  • Minimum 1 year of health training experience.

Knowledge, Skills and Abilities

  • Demonstrated computer skills including Microsoft office. Ability to effectively operate multi-line telephone console, and to use professional telephone etiquette is required.
  • Highly effective written and verbal communication skills including courtesy, resourcefulness and efficiency in answering questions, giving directions, and explaining community or department procedures and processes are necessary.
  • Self-motivated, and confident worker.
  • Highly effective critical thinking and interpersonal skills, with the ability to identify barriers and promote desired outcomes.
  • Excellent organizational and work management skills a must.
  • Ability to adapt to change.
  • Ability to multitask at any given time while maintaining strong attention to detail.
  • Ability to work in a productivity driven environment a must.
