Position Summary
A full-time Primary Care Physician is sought to support a growing eldercare program that emphasizes community-based care , whole-person treatment , and a collaborative interdisciplinary approach . This position focuses on serving medically complex senior patients through comprehensive in-person assessments, care coordination, and oversight of medical treatment across settings including homes, nursing facilities, and hospitals.
Key Responsibilities
•
Conduct in-person comprehensive assessments for new and existing patients.
•
Coordinate participant care plans in collaboration with a full Interdisciplinary Team (IDT).
•
Oversee care for patients admitted to nursing homes and hospitals (including short-term, long-term, and acute placements).
•
Monitor and manage transitions of care (e.g., admits and discharges).
•
Guide and supervise Nurse Practitioners and other team members as needed.
•
Visit participants in nursing homes as needed, based on individual care plans.
•
Participate in rotating night and weekend call coverage .
•
Support seamless coordination with community physicians and other specialists.
Minimum Qualifications
•
Doctorate in Medicine (MD or DO) from an ACGME-accredited residency program
•
Board Certification in Internal Medicine , Family Practice , or Geriatrics (preferred)
•
Active California Medical License (unrestricted)
•
Active DEA license and state-controlled substance registration
•
Eligible for enrollment as a provider in Medicare and Medi-Cal
