Core Responsibilities
- Conduct thorough initial and ongoing nursing assessments of patients in their home environments to evaluate health status and identify care needs
- Develop, implement, and regularly update individualized care plans in collaboration with physicians, therapists, and other interdisciplinary team members
- Administer medications, wound care, injections, and other prescribed treatments according to physician orders and established clinical protocols
- Monitor and document patient progress, vital signs, and responses to treatment accurately and in a timely manner
- Educate patients and family members regarding disease management, medication adherence, nutrition, and safety practices
- Coordinate with case managers, social workers, and community resources to ensure continuity and comprehensiveness of patient care
- Recognize and respond appropriately to changes in patient condition, escalating concerns to physicians or supervisors when clinically indicated
- Maintain complete, accurate, and compliant clinical documentation in the electronic health records system
- Participate in quality improvement initiatives and adhere to all regulatory and accreditation requirements governing home health services
Must-Have Requirements
- Active and unrestricted Registered Nurse license in the state of Texas
- Minimum of two years of clinical nursing experience, preferably in home health, medical-surgical, or a related specialty
- Current CPR and Basic Life Support certification
- Valid driver's license and reliable personal vehicle with appropriate insurance coverage
- Strong clinical assessment and critical thinking skills
- Excellent verbal and written communication abilities
- Proficiency with electronic health records and standard clinical documentation platforms
- Ability to work independently, manage a caseload efficiently, and prioritize competing responsibilities
- Demonstrated commitment to patient dignity, cultural sensitivity, and ethical nursing practice
