Key Responsibilities
- Develop case plans based on clients' social, emotional, and healthcare needs
- Conduct assessments to identify challenges and help clients access resources
- Collaborate with healthcare providers, social services, and community organizations to arrange services •
Provide ongoing support and adjust care plans as necessary
- Assist clients with overcoming barriers, such as financial, housing, or healthcare challenges •
Advocate for clients in legal, healthcare, or educational settings

- Maintain detailed and confidential records in compliance with regulations
Qualifications
•
Bachelor’s degree in Social Work (BSW) required; MSW preferred
•
3+ years of experience in social work or case management
- Strong understanding of social work principles, ethics, and confidentiality standards
- Excellent communication, problem-solving, and time-management skills •
Ability to work independently and collaboratively within a team
- Experience with healthcare systems, community resources, and legal advocacy
- State licensure as a Social Worker (LCSW or equivalent) preferred
