Nurse Practitioner (NP) Responsibilities
•
Conduct real-time clinical assessments of patients in long-term care facilities
•
Develop and implement treatment plans, prescribe or recommend behavioral medications, and provide medication management
•
Collaborate with psychiatrists (as required by state law), psychologists, physicians, and clinical pharmacists
•
Document patient encounters in the Electronic Health Record (EHR) system
•
Provide virtual in-service education to facility staff and attend team meetings as needed
•
Offer consultation on nursing practices and psychopharmacology, including medication orders when appropriate
•
Participate in case reviews, utilization reviews, and handle urgent clinical issues in collaboration with facility staff
Nurse Practitioner (NP) Qualifications
•
Advanced Practice Nursing degree from an accredited institution
•
Valid Nurse Practitioner license in the state of practice (or in process); willingness to obtain multi-state licensure if needed
•
Controlled Substance License and DEA Certificate (or in process)
•
Ability to enter into collaborative agreement or attestation with a physician, as required by state law
•
Meets credentialing standards for healthcare facilities and insurance providers
•
Eligible for Medicare and Medicaid credentialing
•
Geriatric and/or psychiatric experience preferred
•
Proficient in using digital tools, including EHR systems, Microsoft Outlook, and Excel
