Hospice RN Case Manager – Meaningful Care + Supportive Team | North Suburbs (IL)

This opportunity is with a well-established hospice team in the North Suburbs of Chicago that is focused on creating a more sustainable and supportive environment for nurses doing this important work.

The role

As a Hospice RN Case Manager, you'll manage and coordinate care for patients in a home-based setting, working closely with an interdisciplinary team to ensure comfort, dignity, and quality of life.

You'll be responsible for

Managing patient care in hospice settings

Coordinating with physicians and care teams

Supporting patients and families through all stages of care

Providing leadership and guidance within the care team

Schedule & structure

Monday–Friday schedule (daytime hours with flexibility in start/end times)

  • Limited on-call expectations: •

1 weekend day per month (scheduled visits + overnight on-call)

2 weekday on-call shifts per month

Dedicated after-hours RN team in place for additional support

Additional compensation provided for on-call coverage

2 holidays per year + floating holidays

Territory

Local travel within a defined service area

Focus on maintaining reasonable coverage and minimizing unnecessary travel

What they're looking for

Active RN license in Illinois

1+ year of hospice or home health experience

Experience in case management or care coordination

Strong clinical judgment and ability to work independently

A passion for patient-centered, end-of-life care

Certifications

AHA BLS (required)

AHA ACLS (required)

Compensation

$70,000 - 90,000 annually

Additional stipends for on-call coverage

Openings available in Elmhurst, McHenry, Mount Carmel, Mount Pleasant, Mount Vernon, Northbrook, & Wheeling, IL
