Part-Time Personal Support Worker Community/Home Care in Hamilton

About the Role

HealthOPM is seeking compassionate and dedicated Personal Support Workers (PSWs) to join our growing team in Hamilton, ON . In this rewarding community-based, home-to-home role, you'll make a meaningful difference in the lives of seniors by supporting their daily routines, well-being, and independence.

Location: Hamilton, ON (Community Home-to-Home)

•

Shift: 5:00 PM 12:00 AM

•

Pay Rate: $20/hour

•

Position Type: Part-Time

•

Transportation: Must have a valid drivers license and reliable vehicle

Key Responsibilities

•

Assist clients with activities of daily living (ADLs) and personal care

•

Provide emotional support and meaningful companionship

•

Observe, document, and report changes in health or behaviour

•

Support clients with mobility, hygiene, and nutritional needs

•

Collaborate with nurses and other healthcare professionals

•

Follow individualized care plans and maintain accurate charting

•

Adhere to infection prevention, safety, and confidentiality protocols

Requirements

•

Valid PSW Certification (Ontario)

•

Recent Vulnerable Sector Check (within 12 months)

•

Clear 2-Step TB Test

•

Valid drivers license and reliable vehicle

•

Strong communication and interpersonal skills

•

Experience with dementia care and GPA/NVCI training (preferred)
