Home Health Occupational Therapist (OT)

As a Home Health Occupational Therapist , you’ll provide skilled, one‑on‑one therapy services in the comfort of patients’ homes, helping them regain independence with daily activities and safely navigate their environments. You’ll assess patient needs, develop individualized treatment plans, deliver evidence‑based interventions and educate patients and caregivers to support functional independence and quality of life at home.

Whether you’re experienced in home health or transitioning from hospital, outpatient, acute care or skilled nursing settings, this role offers the opportunity to practice at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationship.

Minimum Qualifications

Must have a minimum of one (1) year experience

Home healthcare experience preferred

Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.

Registration by the NBCOT and registration to practice as an Occupational Therapist in state of practice if state has registration practices.

Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agency’s clients and their families

Must have current Driver’s license and reliable transportation to and from work site.
