Department Overview

The Psychiatric and Psychological departments provide services on both the Minneapolis and St. Paul campuses, with limited services at Woodwinds in Woodbury and Children's West in Minnetonka. Specific services include individual and family therapy, psychological assessment, neuropsychological assessment, and psychiatric evaluation and medication management (Available to children who are already patients of Children's mental health, pediatric, or subspecialty clinics.).

Position Summary

The Clinic Case Coordinator works closely with the clinicians to assist patients with their non-medical needs related to care coordination. The role would interface with patients, parents, referring clinicians, and community partners to concordant communication to and from the clinical team. The role reports to the department manager and works closely with the clinical team. The role works both independently and in conjunction with the clinician team to address complex patient needs in the outpatient clinic and inpatient unit.

  • Bachelor's degree *Two years' experience in a health care setting.

Preferred: *Pediatrics experience preferred. *Knowledge and understanding of health diagnoses, critical thinking skills and psychosocial assessment skills. *Knowledge of hospital and community resources. *Computer knowledge and ability to effectively utilize and document patient information in the electronic medical record. *Knowledge of medical terminology. *Demonstrated customer service and organizational skills. *Effective listening skills. *Ability to show empathy. *Applies cultural competence when working with individuals and teams from diverse backgrounds. *Knowledge of insurance requirements. *Ability to interact with various cultures served by Children's in a culturally competent manner.

The posted salary represents a market competitive range based on salary survey benchmark data for similar roles in the local or national market. Annual salaries displayed are based on full-time employment (40 standard hrs per week). Annual salaries for part-time positions will be prorated based on the employee’s scheduled weekly hours in relation to the full-time standard. When determining individual pay rates, we carefully consider a wide range of factors including but not limited to market indicators for the specific role, the skills, education, training, credentials and experience of the candidate, internal equity and organizational needs.

In addition to your salary, this position may be eligible for medical, dental, vision, retirement, and other fringe benefits. Positions that require night, weekend or on-call work may be eligible for shift differentials or premium pay.
