As a member of our Palliative Care team, you will

• Provide expert palliative care consultations for patients with serious and complex illnesses
• Partner with patients, families, and interdisciplinary teams to align care plans with patient goals and values
• Support symptom management, advance care planning, and shared decision-making
• Collaborate with physicians, nurses, social workers, chaplains, and other healthcare professionals
• Help expand access to compassionate, high-quality palliative care services

Ideal Candidate

We are looking for a Nurse Practitioner who is

• Passionate about caring for patients with serious illness
• Skilled in communication, symptom management, and goals-of-care conversations

• Committed to providing compassionate, evidence-based care

Experience in palliative care, hospice, oncology, geriatrics, critical care, or another related specialty is highly valued.
