The Psychiatric Mental Health Nurse Practitioner Team Lead provides psychiatric care while supporting clinical leadership, care coordination, workflow optimization, and behavioral health program development. This role balances direct patient care with operational and administrative responsibilities, collaborating with IDTs and WelbeHealth PACE Physicians to deliver comprehensive, participant-centered care under the direction of the Medical Director.

On the day-to-day, you will
- Provide psychiatric consultation, illness evaluation, counseling, and treatment for participants in collaboration with PACE Physicians, Community-Based PACE Physicians, and the IDT to develop a comprehensive care plan for each participant
- Collaborate with Physician and the IDT on discharge planning from acute or long-term care settings
- Partner with the PACE Physician or Community-Based PACE Physician to refer participants to a psychiatrist as indicated, as well as manage participant care in nursing homes, making regular visits as dictated by WelbeHealth standards and participant need
- Provide clinical leadership and mentorship to Psychiatric Nurse Practitioners and Behavioral Health team members
- Serve as the first point of contact for discussing complex cases and pre-enrollment decisions
- Support onboarding, training, and professional development of Behavioral Health Clinicians, and participate in quality improvement initiatives, chart reviews, and clinical audits to promote high standards of care
- Assist with the development, implementation, and refinement of clinical workflows, protocols, and best practices, as well as partner with operational and clinical leadership to address program needs, service gaps, and care delivery improvements
Job requirements include
- Master’s degree in nursing, with a psychiatric/mental health emphasis from an accredited college or university
- Certified as a Psychiatric Mental Health Nurse Practitioner
- Active Psychiatric Mental Health Nurse Practitioner state license without restrictions
- State CDS Certification, NPI, and DEA License
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Minimum of four (4) years of experience working with frail or elderly populations
- Minimum of two (2) years working within the PACE model preferred
- Prior experience serving as a functional lead or subject matter expert preferred

Compensation Offering $168,895.66 - $232,231.53 USD
