Job description

We are working closely with local authority in Liverpool to assist with the appointment of an Outreach Family Support Worker , on a 3 -months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Summary

The Outreach Family Support Worker provides high-quality, outreach and home-based Early Help support to children, young people, and families using a whole-family approach. Working within Liverpool City Council's Early Help Service, the postholder manages an allocated caseload of complex Level 2 and Level 3 Early Help cases, delivering targeted interventions that improve outcomes, strengthen family resilience, and prevent escalation to statutory social care services.

Responsibilities

  • Hold and manage an allocated caseload of complex Early Help cases, providing intensive outreach and home-based family support.
  • Undertake comprehensive Early Help Assessments to identify strengths, needs, risks, and appropriate interventions.
  • Act as the Lead Professional, coordinating support and delivering effective multi-agency plans for children, young people, and families.
  • Support families stepping down from Children's Social Care and provide continuity of support during transitions between statutory and Early Help services.
  • Apply the Signs of Safety framework and other outcome-focused practice models to support sustainable family change.
  • Contribute to service improvement, quality assurance, and the development of Early Help practice.

Qualifications

  • Level 3 qualification in Health and Social Care, Childcare, Family Support, Youth Work, Early Years, Community Work, or a related discipline, or equivalent relevant experience.
  • GCSEs (or equivalent) including English and Mathematics.

Essentials

  • Experience working directly with children, young people, and families within Early Help, family support, children's services, youth services, education, health, or community settings.
  • Experience supporting vulnerable children, adolescents, and families experiencing complex needs, including behavioural, emotional, mental health, safeguarding, domestic abuse, substance misuse, or family breakdown.
  • Experience maintaining accurate electronic case records, assessments, and reports.
  • Experience undertaking Early Help Assessments and coordinating multi-agency support plans.

  • This role requires an Enhanced DBS
