Manages and oversees patient navigation workflow, ensuring minimal fragmentation of care and coordination or services, including providing quality customer service; resolving any issues as they occur; and collecting statistical information and reporting to the management team to meet strategic goals and enhance business practices.
Duties & Responsibilities
- Assist the Cancer Prevention and Survivorship (CPS) Clinic Coordinator with developing, maintaining, and implementing the Cancer Prevention and Survivorship clinic and program.
- Follows guidelines and standard operating procedures (SOP); including maintaining a list of social services, community/patient-advocate groups, educational resources, and point of contact for various Oncology specialty providers (e.g. radiation oncology, surgical oncology, plastic surgery, medical oncology, etc.).
- Provides support to physicians, nurses, referral staff, and patients to ensure quality and efficient patient processing and care.
- Assist the CPS Clinic Coordinator in coordinating evaluation for supportive services, including social services, physical therapy, psycho- oncology.
- Serves as liaison between various clinics, hospitals, patient advocacy centers, and UI Cancer Center Clinical Trials Office (CTO) for all available oncology clinical trials; directing patients to the CTO for potential participation.
- Oversees all patient orders requested by provider to ensure timely coordination and continuum of care.
- Serves as the point person for all issues related to patient information, record retrieval and distribution, investigating and ensuring all guidelines are followed by staff; using judgment to escalate issues as appropriate.
- Assist patients with referral scheduling, insurance coverage and issues, and account/billing issues.
- Maintain patient records, including assessments, orders, tests, referrals, and appointment scheduling.
- Responsible for data collection, entry, and management of the Cancer Prevention and Survivorship Clinical Program databases.
- Utilizes databases to run reports as requested by administrative and clinical leaders.
- Communicates and keeps senior leaders apprised with unit operations, issues, and accomplishments.
- Perform other related duties and participate in special projects as assigned.
Minimum Qualifications Required
- Any one or any combination totaling five (5) years (60 months), from the categories below
- College education with course work in a business medical field such as nursing assistant, medical assistant, or medical secretary and/or business fields such as business, finance, public administration, or social science, as measured by the following conversion table or its proportional equivalent:
- 30 semester hours equals one (1) year (12 months)
- Associate’s Degree (60 semester hours) equals eighteen months (18 months)
- 90 semester hours equals two (2) years (24 months)
- Bachelor’s Degree (120 semester hours) equals three (3) years (36 months)
- Work experience or ancillary service experience comparable to the Medical Office Specialist level or other positions of comparable responsibility.
Preferred Qualifications
- Familiarity with scheduling and coordinating patient appointmetns for office visits, screening, and diagnostic tests.
- Proficiency in electronic medical records (EMR) systems, scheduling platforms, and stand office software.
- Experience with tracking patient appointments, follow-up, an documentation to support care coordination efforts.
