The Staten Island Workforce1 Career Center is partnering with Person-Centered Care Services (PCCS) to find qualified candidates for a Community Health Worker in Staten Island, New York. Person-Centered Care Services is a nonprofit organization that creates social change by supporting people with disabilities on their journey to self-identity and acceptance. Position Overview

The Community Health Worker serves as a bridge between the community and healthcare systems by helping individuals and families access healthcare and social services. This grant-funded position provides education, resources, and ongoing support while assisting participants in navigating available services. The role also develops community partnerships, monitors participant progress, and supports grant reporting and compliance activities to improve access to care and reduce health disparities.

This position is located in Staten Island, New York.

Key Responsibilities

  • Conduct community outreach and build relationships with community members, organizations, and stakeholders.
  • Manage a caseload and complete assessments to identify individual needs and goals.
  • Assist individuals in navigating healthcare and social service systems.
  • Maintain accurate records and collect data to support grant reporting requirements.
  • Monitor participant progress and provide ongoing follow-up and support.
  • Connect individuals with healthcare providers, social services, and community resources.
  • Participate in community events to promote available services and strengthen partnerships.
  • The position may involve assisting with toileting, hygiene, and lifting based on the needs of the individuals supported.
  • Complete required trainings and support incident reporting in accordance with OPWDD Part 624 regulations and New York State Justice Center requirements.

Requirements

  • High school diploma required.
  • Associate degree in a related field (such as Social Work or Mental Health) preferred.
  • At least 1 year of experience managing a caseload required.
  • 1 year of experience working with community health partners and referral systems preferred.
  • 1 year of experience developing and maintaining community partnerships preferred.
  • Valid driver's license required.

Pay Range

USD $24.03 - USD $24.03 /Hr.
