Key Responsibilities Conduct detailed assessments, diagnosis, and management of patients with chronic diseases. Develop and implement individualized care plans aimed at disease control and prevention of complications. Provide patient education to promote self-management and adherence to treatment regimens. Collaborate with healthcare professionals to coordinate comprehensive care and optimize outcomes. Monitor patient progress and make adjustments to treatment plans as necessary. Maintain accurate and thorough medical documentation in compliance with regulatory standards. Participate in continuing medical education and stay updated with advances in chronic disease management.

Medical degree specializing in Internal Medicine or equivalent. Certification or specialization in chronic disease management is preferred. Valid medical license to practice in Abu Dhabi/UAE. Proven clinical experience in managing chronic diseases within internal medicine settings. Strong diagnostic, clinical, and patient education skills. Excellent communication and teamwork abilities. Commitment to providing patient-centered care and continuous professional development.
