An exciting opportunity has arisen for a Discharge Planning Assistant to join the team.
The Discharge Planning Team is a 7 day service based at Queen Alexandra Hospital. Reporting to the Clinical Lead, this role will work very closely with allocated wards taking responsibility for supporting the ward staff in managing all discharge pathways and taking on complex cases, under the instruction of Senior Discharge Planners.
You will be a vital part of the hospital’s discharge service working alongside Hampshire and Isle of Wight Healthcare, Hampshire County Council and Portsmouth City Council to ensure a smooth and safe discharge process for our patients.

Essential
- Computer literacy and Keyboard Skills.
- Experience / Understanding of the healthcare setting.
- Educated to NVQ level 2 / 3, BTEC and knowledge about a range of Trust policies and management procedures
- Be able to learn and participate in Portsmouth Hospitals University NHS Trust computer systems.
Desirable
- Health care experience within a hospital setting.
- Experience in managing discharge planning processes.
