Social Worker – Collaborative Care, The Everett Clinic

We are looking for a dedicated, motivated Social Workers to join our Primary Care Team to provide collaborative supported care for their patient behavioral health care needs. Our Social Workers work side by side with Primary Care clinicians to make huge impacts and improve patients’ lives.

Our Social Workers

•

Collaborate with Primary Care providers, Behavioral Health professionals, Psychiatrists, schools, and community resources to help provide wrap around care for our patients

•

Have a patient case load of 65-70 patients

•

See a varied patient demographic caring for patients of all ages

•

Have dedicated patient care time as well as administrative time

Minimal qualifications

•

Education: Bachelor’s or master’s degree from a CSWE accredited school of Social Work

•

Experience: 3 years of progressively responsible and directly related work experience in a healthcare setting required.
