DEPARTMENT : Administration POSITION : Intellectual and Developmental Disabilities Licensed Practical Nurse STATUS : Full-Time; 37.5 hours / week SCHEDULE / HOURS : M-F daylight with some evening and weekends to meet the needs of the clients LOCATION : Wilkinsburg ( 75% community, 25% office)
Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania.
Responsibilities
The Intellectual and Developmental Disabilities (IDD) Licensed Practical Nurse (LPN) provides holistic, person-centered nursing care to individuals with Intellectual and Developmental Disabilities (IDD), autism spectrum disorder, and behavioral health diagnoses. This role supports individuals across multiple settings,
including facility-based programs, community activities, Life Sharing, and community living homes with as needed support provided in a clinical setting. The nurse works collaboratively with interdisciplinary teams to promote health, safety, independence, and quality of life while ensuring services align with medical best practices and person-centered values.
- Demonstrates and reflects a commitment to the mission and values of Milestone by serving others with respect, dignity, and compassion.
- Assists all Directors, supervisors/managers and staff in promoting and maintaining optimal health for the individuals in the program through:
- Regularly reviewing individuals’ medical records, including but not limited to the Medication Administration Record (MAR), Individual Support Plan (ISP), Annual Physical, Health Care Contact records, lab reports and coordinating/providing health care services in agency programs as needed.
- Contributing, updating, and monitoring implementation of Nursing Care Plans through scheduled routine visits to Residential and Life Sharing homes, including observation and baseline evaluation of individuals with prompt appropriate medical intervention as needed.
- Assisting with implementation and oversight of Health Risk Screening Tool (HRST) considerations (including but not limited to assessments, treatments, training).
- Participating in hospital and skilled facility discharge planning meetings as needed for individuals who have been hospitalized/placed and ensuring that discharge instructions are communicated to staff.
- Participating in individuals’ meetings (ISP, Supports Intensity Scale (SIS), inpatient/Rehab Plan of Care, etc.) as needed.
- Coordinating, participating, and monitoring required routine medical and psychiatric appointments including communicating health information to medical personnel, as needed.
- Guiding, supporting and coaching Community Living Managers, staff and families in effectively communicating with medical professionals.
- Collaborate with Community Living Managers, physicians, pharmacists, and families as necessary to address matters related to medications and their administration.
- Assisting in identification and development of educational resources for individuals and families that support health and wellness and provision of these to programs in a format which can be shared with individuals and families.
- Assisting in the provision of support and education/training to all supervisors/managers and Direct Support Professionals (DSPs) related to the clients’ medical diagnoses; prescribed medications; dietary guidelines and restrictions; mobility limitations, positioning needs and emergency health protocols.
- Alerts and reports identified concerns and risks to the Director of Nursing, Senior Director of Residential Programs and relevant Department Director in a timely manner.
- Provides coverage for Behavioral Health nursing staff as needed.
- Understanding and compliance with Parters For Quality Corporate Personnel Policies & Procedures and Milestone specific Personnel and Operational Policies & Procedures.
- Other duties as assigned.

Qualifications
- Licensed Practical Nurse with at least 2 years of nursing experience preferably in Rehabilitation and/or Emergency Nursing and/or working with individuals with intellectual disabilities and/or Autism
- Current Pennsylvania LPN License
- Complete and maintain Department of Human Services (DHS) Medication Administration & Practicum Observer Certification within first year
- Act 33, 34 and FBI Clearances
- Valid PA, OH, or WV Driver’s license and acceptable driving record
