Job description

Akicita Federal is hiring a Nurse Practitioner to deliver high-quality outpatient primary care to Yankton Sioux Tribal Members (Ihanktonwan Nation) through the Indian Health Service in Wagner, South Dakota. This role directly serves a rural community where patients may travel up to 100 miles round-trip to access care, making consistent, skilled clinical presence genuinely life-changing. You will work within a full-service ambulatory health center staffed with pharmacy, radiology, laboratory, dental, behavioral health, and specialist clinic support.

  • Diagnose and treat a broad range of acute and chronic conditions for a diverse patient panel spanning newborns to geriatric adults, averaging 10–16 patient visits per day
  • Provide outpatient mid-level medical services across extended clinic hours, Sunday through Saturday, 7:00 a.m. to 11:00 p.m., including weekends and federal holidays
  • Document clinical encounters accurately and in a timely manner in accordance with facility medical staff bylaws, rules, and applicable regulatory standards
  • Coordinate care with on-site services including pharmacy, public health nursing, behavioral health, pain management, and visiting specialist clinics (OB/GYN, cardiology, nephrology)
  • Participate in the facility's quality improvement program, including ongoing monitoring and evaluation of the quality and appropriateness of care delivered
  • Maintain clinical practice consistent with CMS standards, governing body bylaws, and all applicable state and federal licensure requirements
  • Collaborate with facility leadership and support staff to ensure uninterrupted outpatient department coverage for approximately 7,500 patient visits per month

What sets candidates apart

  • Prior experience practicing in an Indian Health Service, tribal health, or Federally Qualified Health Center (FQHC) setting
  • Familiarity with the health needs and cultural context of American Indian and Alaska Native patient populations
  • Experience in a high-volume rural or critical-access outpatient clinic environment
  • Comfort with extended-hours or shift-based scheduling models, including evenings and weekends
  • Background working alongside integrated care teams that include behavioral health, pharmacy, public health nursing, and telemedicine services
