Job Summary
The Community Health Worker (CHW) Intern participates in a six (6) month rotational program within the Community Health departments to provide exposure and build awareness to optimal community resources that are tailored to patient needs. This position engages in community health department rotations, completing assigned projects under the guidance of the supporting manager to gain exposure to healthcare, organizational practices, and community health resources. The CHW Intern will learn to utilize community resources and establish connections with providers to identify suitable medical, mental health, and dental homes for uninsured and under-insured populations. The CHW intern will rotate and travel across Memorial Hermann locations within the greater Houston area. The rotation schedules, program terms, and assignments are subject to change based on program availability and organizational needs. This role typically reports to a Manager within the Community Health/Community Benefits department.
Job Description
Minimum Qualifications
Education: High school diploma or GED preferred.
Licenses/Certifications Community Health Worker Certification preferred.
Experience / Knowledge / Skills
•
Effective oral and written communication skills.
•
Entry-level computer software skills – Microsoft Word, Excel, and PowerPoint.
•
Demonstrates critical thinking, sound judgement, and effective time and task management.
•
Strong coordination and organizational skills.
•
Ability to work collaboratively with other professionals in fast-paced, service oriented environment.
•
Ability to navigate ambiguity or flexibility in different work environments.
•
Professional conduct in all work-related and emergent situations.
Principal Accountabilities
•
Participates in a structured six (6) month rotational program across various functions of the Community Health and Community Benefits team; including but not limited to Community Resource Centers, Inpatient Navigation, ER Navigation, external nonprofit partner agencies.
•
Shadows, supports, and learns from certified Community Health Workers (CHWs) to gain exposure to best practices in patient engagement, community resources, and organizational practices.
•
Shadows and assists with conducting intake, needs & eligibility assessments, and referrals of patients to appropriate community resources under staff supervision.
•
Participates in team meetings, trainings, and professional development opportunities offered throughout rotation under guidance of the supporting manager.
•
Rotates working locations across community health programs at Memorial Hermann locations across the greater Houston area.
•
Demonstrates reliability and timeliness by adhering to scheduled work hours and rotation assignments; working hours may fluctuate based on program needs or rotation location and are determined by program manager.
•
Completes a project and/or presentation at end of rotation that demonstrates knowledge obtained during experience.
•
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
•
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
