Be a Bridge to Better Health – Join Us as a Community Health Worker Schedule: Monday - Friday 8:00am-5:00pm. 40 hours weekly.

As a Community Health Worker , you’ll serve as a vital link between individuals and the services they need to thrive—connecting people to healthcare, social services, and community resources. Through education, advocacy, and ongoing support, you’ll help individuals take control of their health and navigate systems that may otherwise feel overwhelming. This role directly supports our mission to improve lives and promote wellness in the communities we serve.
