As the Manager of Clinical Practice, here's a big-picture view of what you'll do:
- Provide clinical leadership and oversight of all home health personnel and patient care services delivered by the agency.
- Coordinate patient admissions, referrals, scheduling, and clinician assignments to ensure efficient, high-quality care delivery.
- Oversee interdisciplinary patient care while ensuring individualized plans of care are developed, implemented, evaluated, and updated according to physician orders and patient needs.
- Ensure timely coordination of services and communication among physicians, clinicians, patients, caregivers, referral sources, and community partners.
- Supervise, mentor, and support licensed clinicians and paraprofessional staff while promoting clinical excellence and accountability.
- Monitor clinical documentation for accuracy, timeliness, completeness, and compliance with Medicare regulations, accreditation standards, and agency policies.
- Lead quality assurance activities and participate in the agency's Quality Assessment and Performance Improvement (QAPI) program.
- Utilize clinical judgment to support positive patient outcomes while effectively managing an assigned patient population.
- Ensure patients, families, and caregivers receive education that promotes independence, safety, and achievement of care goals.
- Collaborate with the interdisciplinary team to facilitate continuity of care, care transitions, and discharge planning.
- Support emergency preparedness initiatives and agency compliance programs.
- Maintain confidentiality of Protected Health Information (PHI) in accordance with HIPAA regulations.
- Participate in Administrator/RN On-Call rotation and provide clinical support during agency operating hours.
- Foster a positive, collaborative culture focused on clinical excellence, customer service, and patient satisfaction.
A few must-haves for our Manager of Clinical Practice
- Graduate of an accredited school of nursing.
- Current, unrestricted Registered Nurse (RN) license in South Carolina.
- License must be active and in good standing.
- Minimum of three (3) years of recent home health experience preferred.
- Minimum of two (2) years of clinical leadership or management experience preferred.
- Current CPR certification
- Thorough knowledge of Medicare Conditions of Participation, home health regulations, documentation standards, and quality improvement processes.
- Strong leadership, organizational, and clinical decision-making skills.
- Experience supervising interdisciplinary healthcare teams.
- Excellent written, verbal, and interpersonal communication skills.
- Ability to effectively prioritize multiple responsibilities in a fast-paced environment.
- Strong customer service skills with the ability to build relationships with patients, families, physicians, referral partners, and clinical staff.
- Proficiency with electronic medical records (EMR) systems and Microsoft Office applications.
- Demonstrated professionalism, integrity, sound judgment, flexibility, and commitment to delivering exceptional patient care.
