Our reputation for excellence is built on great choices and empowered leadership, and fosters a team that’s focused on evidence based practice, continual learning and exceptional quality of care Job Details The Community Health Worker is responsible for identifying high risk prenatal and postpartum women and assist these individuals in gaining access to primary and preventive health services including prenatal care, PCAP, WIC, and other essential and supportive services as needed. Requirements: High School Diploma, equivalent or relative experience. Valid New York State Driver's license, personal car to carry out employment duties and an active automobile insurance policy. Basic computer skills. Good writing and verbal skills. Spanish or Creole speaking a plus Salary Range Starting from USD $22.00/Hr. This rate serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
